Home dietetics/ tele-dietetics services are offered to elders who require dietary advice within the convenience of a home setting.

This service includes the following:

• Provide medical nutrition therapy
• Assess nutritional well-being of elders and implement specific diet strategies for the management of chronic conditions such as diabetes and acute illnesses, or injuries
• Provide dietary counselling and customise a nutrition care plan for elders who are unable to take their meals by mouth and have to be fed through a tube
